What Is Rhinophyma?

For patients who have reached the established phase of the condition, surgery is the only reliably effective treatment.

Rhinophyma is a specific form of advanced rosacea affecting the nose. In rosacea, the small blood vessels of the facial skin become persistently dilated and the skin chronically inflamed. In a proportion of patients, over time this leads to progressive overgrowth of the sebaceous glands and underlying fibrous connective tissue — most characteristically affecting the nose. The skin of the nose gradually becomes thickened, bumpy, and irregular, with prominent pores and an increasingly enlarged, bulbous shape.

The condition is almost entirely confined to men and typically develops in middle age or later, slowly progressing over years. It is not related to alcohol consumption, despite a common misconception — though alcohol-related facial flushing can aggravate rosacea and contribute to worsening skin redness alongside established rhinophyma. Once established, rhinophyma does not resolve spontaneously, and the skin does not return to normal with medical treatment alone.

Some research suggests heavier alcohol intake may be associated with more severe rhinophyma in those already prone to it, likely because alcohol triggers facial flushing and vasodilation — but alcohol alone doesn't cause the condition.

The procedure is usually done under local anaesthetic, so you shouldn't feel pain during treatment, and discomfort afterwards is generally mild and manageable with simple pain relief. Immediately after treatment, the skin surface will look red, raw, and may weep for the first week or so — this is the normal, expected appearance as new skin regrows over the reshaped contour, not a sign of anything going wrong.
